1. To download the Family Information Submission Form (submit.pdf) "Right Click Here" and select "Save link as".
a) Open the (submit.pdf) with your browser and enter your family information.
b) After entering your family information, click on the Printer Icon on the top right side of your browser.
c) In the Destination, select "Save as PDF" or "Save to PDF" then click the "Save" button.
d) On the next page click the "Save" button.
e) Your completed Submission Form will be saved to the same location where you downloaded the (submit.pdf).
f) Attach your completed Submission Form to an email and send to:
