1. To open the Family Information Submission Form (submit.pdf) online, "Click Here".
a) The pdf form(s) can be filled out online and saved to your computer.
b) Attach your completed and saved pdf form(s) to an email and send to martelandmartell@gmail.com
2. In order to open and edit the "submit.pdf" file you will need Adobe Reader or some other pdf reader.
3. Adobe Reader is available for free and can be downloaded here
